We’re here to make your enrollment process as smooth as possible. This checklist will help you gather everything you need before you begin.
You must be 18 years of age or older to receive treatment at 5280 Human Care Center.

Initial account set up takes on average 5 to 10 minutes, but before you begin enrollment, it helps to have a few key items ready. The checklist below walks you through the documents you will need to provide and personal information you’ll be asked about during your intake. Having everything prepared ahead of time means fewer delays and a smoother start to your program. If you’re missing something, like a court order that hasn’t been issued yet, don’t worry. You can still get started and submit outstanding documents as soon as they become available.
Documentation showing your treatment requirements for DUI classes and/or outpatient therapy from the DMV, court, or probation officer.
Driver’s license, state ID, or passport. We’ll need your driver’s license number for DMV communication.
If you’re transferring from another agency, you MUST provide a copy of your DRS (Discharge Report Summary), or no credit will be given for previously attended classes.
Note: If you haven’t attended court yet or been assigned a probation officer, you must provide this paperwork as soon as it becomes available.

Be prepared to provide honest information about your health and legal history during your intake assessment
Important: Your probation terms will be verified by the courts, probation officer, and/or DMV. Failure to disclose complete information may result in non-compliance and discharge from the agency. Treatment records are confidential—only attendance and payment compliance will be shared with the court.
All classes are held online via Zoom—no in-person visits required. Before you enroll, confirm you have the following:
A device with a camera and microphone A computer, laptop, tablet, or smartphone. A computer or laptop is strongly recommended; it allows you to see all participants on one screen and is the most user-friendly option.
A reliable internet connection You must maintain a stable connection throughout the entire session with both audio and video on.
A private, quiet space You are required to attend from a location that protects the confidentiality of others in your group. A car (stationary), home office, or private room works well. Public spaces, coffee shops, and shared areas are not permitted.
No Zoom account needed. Once enrolled, you’ll receive access to the Client Portal, where you can join sessions directly with one click. A Zoom test link will be provided so you can check your audio and video before your first group.
Once you submit your enrollment form, here’s what to expect:
Your initial enrollment takes approximately 5 minutes to complete online. After submission, you’ll receive an email invitation to your Client Portal within 24–48 business hours.
You’ll then be scheduled for a virtual intake appointment—a one-on-one session with a clinician that takes approximately 45 minutes. This is a confidential psychosocial assessment to understand your needs and determine the right level of care for you. Come prepared to share honest information about your health and legal history. Your intake is conducted via Zoom, just like your weekly classes.
After your intake is complete, you’ll be placed in a group and can begin attending classes.
5280 Human Care Center is a self-pay program. We do not currently accept insurance. Here’s a full breakdown of what to expect:
If your balance reaches $60 or more, you will not be permitted to attend groups until the balance is paid. Maintaining timely payments is required for continued enrollment and compliance.
All DUI education and therapy classes are held online via Zoom and are available 7 days a week, from 7:00 AM to 7:30 PM (Mountain Time). Whether you work early mornings, evenings, or weekends, there’s a session time that fits your life.
Classes are conducted live with a licensed clinician and a small group of peers (not pre-recorded) so you can engage, ask questions, and work through the program in real time. All classes meet Colorado state mandates and are approved by the State of Colorado and the Colorado DMV.
