I got a DUI, now what?

Getting a DUI can be very overwhelming. The client support team at 5280 Human Care Center will help you navigate the treatment portion of your requirement. Understanding how your DUI affects your driving privileges and how the courts determines fines, community, service, and/or jail time is vitally important to ensure your compliance. NO DUI Colorado is a state-funded resource of information to help drivers understand Colorado’s impaired driving laws and how they apply to drivers facing a DUI.

What is required to register?

To register for DUI classes and/or outpatient therapy services at 5280 Human Care Center, clients must meet several requirements to ensure they receive the appropriate level of care. Here are the typical requirements:

Legal Documentation:
    • Court Order or Probation Requirement: Clients must provide documentation showing treatment requirements for outpatient therapy and/ or DUI classes from the DMV, court or probation officer. If you have not attended court and/or been assigned a probation officer, you must provide paperwork as soon as it becomes available.
    • Driver’s License Information: Clients must provide their driver’s license number and/ or photo for any and all communication with the Colorado DMV.
  • DRS Forms: The DRS form is a legal record of your attendance in any prior DUI treatment. ALL clients transferring from other agencies MUST provide a copy of the DRS, or no credit will be given for any classes previously attended.
Personal Information:
  • Identification: Clients must provide a valid photo ID, such as a driver’s license, state ID, or passport.
    • You must be 18 years of age in order to receive treatment at 5280 Human Care Center
  • Contact Information: Clients must keep a current address, cell phone number, and email address on file for communication purposes. Session reminders are sent 24 hours prior to the text and email numbers on file.
Initial Assessment:
  • Intake Form: Clients must complete a comprehensive psychosocial  intake to assess needs and determine the appropriate level of care for DUI services and outpatient therapy.
  • Please Note: It is important to be honest about your health and legal history. The terms and conditions of your probation will be confirmed and verified by the courts, your probation officer and/or the DMV. Failure to disclose complete information may result in you failing to meet your treatment requirements and may result in discharge from the agency for non-compliance.
  • Treatment records are confidential. Only attendance and/or payment compliance will be shared with the court. The exception is when clinical assessment indicates a potential harm to oneself or others.
Financial Requirements:
Consent Forms:
  • Release of Information (ROI): Clients must sign a Release of Information form allowing the agency to share progress and compliance reports with authorized individuals such as probation officers, case managers, or attorneys.
  • HIPAA Compliance Forms: Clients must acknowledge and sign forms related to the privacy and confidentiality of their treatment under HIPAA regulations (Privacy Policy).
  • Current clients: Click here to add/update a Release of Information. A copy of your ROI will be placed in your client portal under documents.
Class Attendance Commitment:
  • Class Schedule Agreement: Clients must commit to attending scheduled classes on time. Clients must understand and agree to the attendance policy, including the consequences of absences or tardiness (Attendance Policy).
  • Group Participation: Clients must agree to actively participate in group sessions and complete any required homework or assignments. Failure to participate could lead to discharge from the agency for non-compliance (Participation Policy).
Technology Requirements:
  • Access to Technology: Clients must have access to a reliable internet connection and a device capable of supporting telehealth sessions (e.g., computer, tablet, or smartphone).
  • Familiarity with Platform: Clients must be able to understand the telehealth platform, Zoom, used for delivery of classes.
  • You do not need a Zoom account. You will log in directly from the client portal under appointments.
  • Audio/ video requirements: Clients are required to maintain audio and video communication throughout the entirety of the group session (Tech Support Policy).
Communication:
  • Contact Information Updates: Clients must agree to promptly inform the agency of any changes in personal contact information, or any updates/ changes to the probation officer, case manager, or attorney.

How do you ensure the privacy and security of my personal information?

5280 Human Care Center is dedicated to protecting the privacy and confidentiality of our clients. Our business model, procedures, and updated release of information policy ensure that all client information is handled with the utmost care and in full compliance with HIPAA regulations. We release information only with either written consent from the client or as required by law. Your personal details will remain secure and confidential.

What is a release of information (ROI)?

To ensure seamless and compliant communication regarding your treatment progress, it is necessary to have a Release of Information (ROI) form on file. This form allows us to share your pertinent treatment details with authorized individuals such as your Probation Officer, Case Manager, or attorney.

  • Client information: All reports and forms include attendance and billing history; no confidential information is disclosed, and will only be released with written consent from the client or as required by law.

Why is it important to keep my information up-to-date on the ROI?

Current Clients: Click here to add/update a Release of Information. A copy of your ROI will be placed in your Client Portal under documents. Accurate information about your Probation Officer, Case Manager, or attorney ensures timely and precise compliance reports, which are crucial for your case. Delays in reporting can negatively impact your progress, so it is essential to inform us immediately of any changes. We require an updated Release of Information to continue providing updates to the Court.

Attendance / No-Show Policy

5280 Human Care Center has a strict Attendance and No-Show Policy.

Consistently attending DUI treatment helps prevent future driving while impaired by alcohol or other drugs (AOD) by changing the pattern of AOD use that caused problems in the past leading to impaired driving. We recognize that life happens, and we understand that things like work, family and other challenges arise in schedules. Your attendance in DUI treatment is required by the court and/or DMV and should be considered both a time and financial priority.

CLIENT RESPONSIBILITY

Absences are to be reported as soon as you know you will be missing a group. You can report an absence here.

All absences excused or unexcused are documented as a no show (ns) if not reported or not attended (na) if they are reported.

A $20 no-show fee is applied to missed groups.

If you have an Affidavit of Enrollment (AOE) for the DMV, attendance is mandatory. Weekly attendance is reported to the DMV and non-compliance with attendance may cause revocation of your driver’s license without warning.

If you are permitted to drive under an AOE, four (4) No-Shows in a row REQUIRE discharge per DMV and/or Colorado State Law.

Inconsistent attendance interferes with your progress, if you are absent three or more times you could be placed on an attendance contract.

If the attendance contract is violated by a no-show, you may be discharged.

If you are discharged for non-compliance with attendance, you may be prevented from re-enrolling with our agency.

Behavior / Intoxication Policy

5280 Human Care Center has a strict Behavior Policy.

BEHAVIOR POLICY
The therapeutic environment encourages growth for clients in changing the pattern of AOD use that caused difficulties in the past leading to impaired driving. Engaging in treatment requires that the therapeutic space is respected. Participants are required to follow all expectations to provide the best experience for all members of our therapeutic community.

CLIENT RESPONSIBILITY
Confidentiality:
Whatever information is shared in the group must remain in the group.

Children, partners, and/or roommates should not be in proximity to the participant in order to maintain confidentiality for others in your group.

A car, coffee shop, work or any public space is not conducive to privacy, and can violate the rights of others.

Respect:
Healthy dialogue is rooted in self-experience, self-reflection and ownership for your own feelings and thoughts.

If you feel you need to challenge someone else’s idea or statement: It is best to utilize “I statements,” such as “When I heard your comment, I found myself getting reactive,” or “I am feeling upset right now, and I am having a hard time naming what is causing this response.”

Before choosing to speak on behalf of someone else, check in with that person first.

If you are aiming to be supportive of someone else, it is still important to speak from your own point of view. Pause and consider if your words accurately reflect what you are truly trying to say.

We are all in the process of growing and our thoughts do not have to be perfectly formulated, however, what we say out loud needs to remain polite and considerate.

You must do your best to maintain a positive attitude and will refrain from being disruptive, hostile, argumentative, uncooperative, or otherwise unbecoming verbally or physically.

If you are upset and/or angry with the Clinician and/or peers, you may address those issues after group via zoom, email, and/or request an individual meeting.

Clinicians will provide one warning for clients whose behavior does not meet expectations.
Individuals who continue to disrespect the therapeutic environment will be removed from the group, not allowed to reenter the group, charged the no show fee, and will not receive credit for the group.

If removed from group for behavior, the incident(s) and your treatment chart/progress will be reviewed and you may be required to meet individually to address the behaviors before being able to return to any group.

Continued problematic behavior and/or lack of therapeutic progress may result in discharge from the program.

If you are discharged for non-compliance with this behavior policy, you may be prevented from re-enrolling with our agency.

INTOXICATION POLICY
5280 Human Care Center has a strict Intoxication Policy about attending groups while under the influence of ANY substance, prescribed or otherwise.

CLIENT RESPONSIBILITY
Participating in group while under the influence of substances impairs one’s ability to engage meaningfully and absorb the material presented. It is also disruptive and triggering to others in the group.

No substances, prescribed or otherwise, should be used within 4 hours of your scheduled group. If you are taking medications regularly, please select a group time at least 4 hours before or after your medication dosage.

If the Clinician suspects you are under the influence, you will be removed from the group, not allowed to reenter the group, charged the no show fee, and will not receive credit for the group.

If removed from group for suspicion of being under the influence, before being able to return to any group, you will be required to attend an individual meeting to determine the next steps.

Attending a group under the influence may result in discharge from the program.
If you are discharged for non-compliance with the intoxication policy, you may be prevented from re-enrolling with our agency.

Participation Policy

5280 Human Care Center has a strict Participation Policy.

POLICY
To ensure the best virtual experience and treatment outcomes, clients are expected to actively engage in therapy—contributing to discussions, sharing relevant information, and collaborating with the group toward their goals. Playing an active role is key to achieving positive outcomes in therapy.

CLIENT RESPONSIBILITY
Clients are expected to have access to reliable technology during the live session. Your webcam must be operational, and you must be visible on camera throughout the entire group.

The Zoom rooms are opened ten minutes prior to the scheduled group time, and no one will be admitted beyond five minutes after the group start time.

To receive credit for the session, you must attend the ENTIRE group, with audio and video on. If you leave the group early, you will not receive credit for the group.

Clinicians may remove individuals from the group due to participation concerns, and if you are removed, a no-show fee will be charged.

To make the most of your group experience, please join from a quiet, private space where you can fully engage -hearing and being heard – without distractions or compromising anyone’s confidentiality. Your presence matters!

To support full participation and meet program requirements, please be seated upright at a desk or table – similar to an in-person setting – to complete writing assignments as needed. Bringing your workbook to every group session is essential for compliance with Colorado’s treatment regulations. Listen to others and share your thoughts, feelings, and experiences. Also, complete all written workbook assignments to examine your behaviors and gain insight.

If your vehicle is the only consistent, private, and quiet place you have available, you MUST adhere to all group policies regarding confidentiality. You cannot have anyone else in the vehicle. You must have your workbook, and the vehicle cannot be moving, with you as the driver or passenger.

The following are not permitted during a group session: cooking, cleaning, childcare, televisions, cell phones, and music.

Failure to comply with the agency participation policy may result in discharge from the program.

Payment policy

5280 Human Care Center has a strict Payment Policy.

Please consider payment of classes a priority and budget accordingly.

We require clients to maintain a valid form of payment on file to ensure timely processing of service fees. Clients set up to Pay Per Class will be charged the day after their group meets.

If your payment does not clear and your balance is $60 or higher, you will not be allowed to attend any group until you have paid the balance.

Failure to comply with the agency payment policy may result in negative discharge from the program and you may not be eligible to re-enroll.

Additionally, if discharged for non-payment, you will only receive credit for classes that have been paid for and your Affidavit of Enrollment (AOE) for the DMV will be revoked.

How do I sign up for autopay?

To ensure seamless and timely payments, we offer a secure and convenient auto-pay option. You may sign up/update your autopay method through the Client Portal under BILLING. If you have any questions or need assistance, please contact our billing department.

Auto-Pay Basics:
  • Sign Up for Auto-Pay: Ensure your payments are always on time by enrolling in our auto-pay system.
  • Update Payment Method: Easily change your credit or debit card information on file.
  • Payment Methods Accepted: We accept all major credit and debit cards.
  • No ACH/Checks: Please note that we do not accept ACH payments or checks.
Get Started with Auto-Pay:
  • Fill Out the Form: You can use the Client Portal to update/add payment information or click here to access the auto-pay enrollment form.
  • Enter Your Information: Provide your credit or debit card details. Credit cards are subject to a processing fee. Bank debit cards do not incur any additional fees.
  • Confirm Your Enrollment: Review and confirm your auto-pay setup.

How do I access the client portal?

You can access the Client Portal here and log in with your credentials. Once you register, you will receive an email invitation to create your account within 24-48 business hours. This invitation will guide you through the registration process, allowing you to set up your username and password.

  • If you have already registered, you can search your email for “5280 Human Care Center (Client Portal)” to find the original email invitation.
  • If you have already logged into the Client Portal, You can do a password reset if you have forgotten your password.

Do I need a Zoom account?

No, you do not need a Zoom account, you can log in directly from the Client Portal under appointments or from the links in the session reminders in your email or text.

ZOOM Instructions
  • Before your first group, you will need to download the ZOOM app.
  • Download the desktop app if you are using a computer or laptop (preferred) or you can use the mobile app to download zoom to your android or apple device.
  • You must use a device that has both audio and video (required).
  • A computer is the most user friendly and allows you to see all participants on one screen.
  • You’ll also want to make sure your device is logged in under your full name, as opposed to “iPhone” or “Bob’s Tablet”.
  • You can use this Zoom test link to check your audio and video connections before logging into group the first time. This will minimize any delays in treatment due to technical difficulties. Here is the ZOOM test link.

How do I obtain proof of enrollment? (As requested by the court, attorney, or PO)

Proof of Enrollment is sometimes needed for the Court, your attorney, and/or probation officer. This is not the same as an Affidavit of Enrollment for the Colorado DMV. Proof of Enrollment reports can be requested through the Client Portal or by filling out this request form.

How can I obtain an Affidavit of Enrollment (AOE)? (Required by the DMV)

An Affidavit of Enrollment (AOE) is required by the DMV to verify your enrollment in our DUI program and to meet DMV compliance requirements. Since Proof of Enrollment reports do not fulfill DMV requirements (they are for the courts, your attorney, or PO only), you will need to request an AOE separately. NEW AOEs are $20 and are submitted on your behalf to the DMV. Approval or denial of your AOE will be placed in your Client Portal.

It is important to note the expiration date on your AOE.

Attendance is mandatory and treatment must be completed by this date or you may face a suspension and/or revocation of your driving privileges by the DMV. If your AOE expires during treatment, you must request a new AOE. Approval is at the discretion of the agency and/or DMV and is based on compliance with treatment requirements.

How can I obtain my progress report?

Progress reports are automatically sent to probation officers (POs) on file at the beginning of each month. If you need a progress report for your attorney or the court (excluding the DMV), Progress reports only contain attendance and compliance information but do not include any notes or other confidential information associated with your case/ treatment.

Progress reports can be requested through the Client Portal or by filling out this request form.

What is a DRS?

A Discharge Summary (DRS) is a legal document that provides detailed information about your attendance during your time in the program.  We issue DRS (Discharge Report Summary) forms only to clients who are transferring, graduating, or otherwise discharging from the agency. Do not complete this form if you want to remain enrolled. If you have completed treatment and/or are requesting to transfer or discharge from the agency, please click here to complete your DRS Request and Exit Interview.

  • DRS forms will be provided within 10 business days of completing your treatment requirements.
  • A copy of your DRS will be placed in your client portal and/or emailed to the email address on file.
  • A copy of your DRS will be provided to your PO with valid ROI and to the DMV with an ROI upon special request.
  • Please note: You will not receive credit for classes that aren’t paid for.
    • If you have an outstanding balance, you will be discharged as incomplete with credit for only those classes which have been paid for.
    • Please make sure to check the Client Portal for any outstanding balance that may delay your discharge.

If you have completed treatment, your DRS will be in the Client Portal. If you no longer have access, you can request a password reset or contact us for assistance.

How do I upload a DRS from another agency to my treatment record?

If you are transferring to 5280 Human Care Center from another agency, we will need a copy of your DRS before you are able to start treatment.

How do I sign up for Level 2 Therapy after graduating from Level 2 Education?

If you have completed Level 2 Education and are a current 5280 Human Care Center client then please visit the Client Portal and look for the “Sign Up for Therapy” document two weeks prior to completion of DUI education.

If you have not completed Level 2 Education the you will need to complete this before registering for Level 2 Therapy.

How do I reinstate my account?

If you were discharged before completing treatment, you may be eligible for reinstatement. Previous issues with absences, payments, and/or policy violations may prevent you from reinstating. Please check your Client Portal for the “Notice of Termination”. If there isn’t a “Notice of Termination”, please submit a new Intake Form.

Can I switch groups?

Switching in and out of groups is disruptive and can affect the privacy, safety, and trust of the established group. This accommodation may be extended as a one-time courtesy for clients who have an unexpected school, work, family, or other schedule conflict that cannot be avoided, however, if you are not able to commit to attending a group on a weekly basis, we cannot guarantee your continued placement in treatment.

Sometimes personalities don’t mesh. If you find that your current clinician is not a good fit, we will work with you to find a suitable match. To request a group switch, please click here

How do I update my contact information?

Once enrolled, updates to contact information, like mailing address, email, and phone number, can be made in the Client Portal.
